New Ownership for Local Geriatric Care Management Company, Serving Seniors

MOUNT CLEMENS, MI – September 16, 2015 – Serving Seniors, a professional geriatric care management company based in Michigan, has a new owner, Misty Delegato, who is also the founder and owner of Relevar Home Care.

Misty Delegato will continue Serving Seniors’ legacy of providing dedicated care management services to families in Michigan and expressed excitement about bringing her aging care experience to bear with Serving Seniors. “Home care and geriatric care management have a significant common goal – exceptional service that allows seniors to age safely and independently in the home environment of their choice. When it comes to navigating the care needs and options for senior loved ones, so many families in our area are in need of help. I want Serving Seniors to continue to be an expert resource for these individuals, while offering them the dedicated support and assistance they need and deserve.”

Services from experienced nurses and social workers at Serving Seniors begin with a comprehensive assessment in the hospital or current living setting to identify care needs and the creation of an individualized plan of care. Serving Seniors also helps older adults establish and maintain an independent lifestyle, assists with transitions to assisted living and nursing home care, serves as a resource for both long-distance and local family members, and offers counseling, education and support.
